Responsibilities
As a case manager in our Emergency Department with CHI health, you
will coordinate the care and services of patients across the
continuum of illness; promote effective utilization; monitor health
care resources; and assume a leadership role with the
interdisciplinary team to achieve optimal outcomes through the
tools of case management. The case manager will perform a wide
variety of high-level patient care activities to include admission
assessment, screen for appropriateness, and development of a
discharge plan. Timely and appropriate discharges will be
accomplished by evaluating the patient's post-discharge needs and
the provision of services necessary to meet those needs. The case
manager must be knowledgeable in community resources and alternate
care facilities.

Qualifications
Education:
BSN or ADN and 5 years of clinical experience
Major Experience:
Three (3) years of clinical nursing experience in the acute care or
medical setting OR ADN and 5 years of clinical experience
Licensure/Certifications:
Registered Nurse (RN) license. BLS.
ACM certification or acquire it within 2 years of hire (eligible to
sit for exam at 1 year of experience in case management.)
