We are seeking compassionate and skilled Registered Nurses to join our dynamic Hospital at Home team in College Station, Texas. As a Hospital at Home Registered Nurse, you will have the unique opportunity to provide personalized care to patients within their homes. You will play a crucial role in assessing clinical and environmental needs, fostering a healing environment, and supporting patients and their families through education and emotional support. KEY RESPONSIBILITIES · Provide high-quality acute care services to patients enrolled in the Hospital at Home program. · Conduct comprehensive assessments and develop individualized care plans tailored to each patient's needs. · Spend additional time at bedside assessing clinical and environmental needs and supporting an overall healing environment. · Act as a key educator and provide ongoing support to patients and their families, ensuring they have the necessary resources and understanding to manage their care at home. · Collaborate closely with interdisciplinary teams to coordinate care and optimize patient outcomes. QUALIFICATIONS EDUCATION - Grad of an Accredited Program MAJOR - Nursing CERTIFICATION/LICENSE/REGISTRATION - Registered Nurse (RN)
