Job Description:
Memorial Hermann is seeking a dedicated and skilled Advanced
Practice Provider (APP) to join our acclaimed health system in
Texas. As a leader in healthcare excellence, we are committed to

clinical innovation. The APP will play a crucial role in delivering
personalized, patient-centered healthcare, working collaboratively
with physicians and other healthcare professionals to assess,
diagnose, and manage patient care across various clinical settings.
Conduct comprehensive patient assessments to diagnose and treat
illnesses, injuries, and other health conditions. Develop and
implement patient management plans, record progress notes, and
assist in provision of continuity of care. Prescribe medication
following the state’s statutes and professional guidelines. Educate
patients and their families on prevention and healthy habits,
contributing to the overall well-being of the community.
Participate in multidisciplinary team meetings to ensure a cohesive
approach to patient care. Stay current with trends in healthcare
and engage in continuous education and professional development.
Strong diagnostic skills and proficiency in creating effective
treatment plans. Excellent interpersonal and communication skills.
Ability to work efficiently both autonomously and in a team-based
setting. High level of professionalism and adherence to ethical
medical practices. Medical Insurance Dental Insurance Vision
Insurance Life Insurance 401k Current, unrestricted license as a
Nurse Practitioner or Physician Assistant in the state of Texas.
Certification from a recognized certifying body such as the ANCC or
NCCPA. Basic Life Support (BLS) and Advanced Cardiovascular Life
Support (ACLS) certifications required.
