SUMMARY: The Social Service Director is responsible for the
provision of resident services to improve and/or maintain their
psychosocial well-being. DUTIES AND RESPONSIBILITIES: The following
represents the essential functions of the Social Service Director's
role. This list is intended to be a general description of duties
and responsibilities; however, it is subject to change according to
the needs of the facility determined by the Administrator and/or
Corporate Designee. The Social Service Director (SSD) is
responsible for the provision of resident services to improve
and/or maintain their psychosocial well-being. The Social Service
Director frequently rounds the building to visit with the residents
to assess psychosocial needs. SSD is responsible for reviewing 24-
hour nursing report to see if documentation suggests need for
psychosocial intervention. The Social Services Director will
maintain a list of residents to see psychosocial clinicians upon
their next visit- medical, psychological, and social workers. The
Social Services Director may seek resources for resident
intervention from outside providers when needed. The Social
Services Director maintains positive work relationships with case
managers and providers. The Social Service Director obtains,
reviews, and keeps copies of resident Level II PASRR and makes
referrals as appropriate/ recommended by the Office of Behavioral
Health (OBH). The Social Services Director may audit and submit
renewals for 142s. The Social Services Director may assist with
completion of admission paperwork. The Social Service Director also
oversees psychiatric, dental, and vision services. The Social
Services Director will complete MDS assessments timely and
accurately. The Social Services Director will assist residents with
goals and needs. The Social Services Director will ensure proper
discharge planning for each resident and document such. The Social
Services Director will contribute to the resident care plan. The
Social Service Director participates Daily QA (morning) Meetings,
Monthly/ Quarterly QA, Safety Committee Meetings, Resident Council
Meetings when requested, Care Plan Meetings, in-service trainings,
continuing education programs, and all other meetings called by
Legacy or licensing entities. The Social Service Director is a
member of the Interdisciplinary Team (IDT Team). The Social Service
Director is expected to be available during an emergency to assist
the facility and assess residents as needed. The Social Service
assists in maintaining quality resident care through the
establishment of quality assurance/performance improvement
monitoring programs (QAPI) and standards. The Social Service
Director assumes puts measures in place to meet regulatory and
legal requirements. The Social Service Director participates in
community events and marketing plans and programs as needed. The
Social Service Director is a resident advocate educating staff on
Resident's Rights. The Social Service Director will make
introductory visits with new residents and employees timely. The
Social Service Director attends new hire orientation program when
requested to greet and brief new employees on the facility mission
and Social Service Director's role in the building. The Social
Service Director may participate in the Ambassador Rounds Program.
Must act in ways that promote professional and positive
representation of the facility to the community. Communicates in a
professional and courteous manner using effective verbal and
non-verbal communication skills with residents, their families,
co-workers, and vendors. Maintains confidentiality of department,
resident, employee information, communication, and documentation as
well as compliance with the Health Insurance Portability and
Accountability Act (HIPAA) regulations. Clocks in and out for all
working/training hours. Working off the clock will not be permitted
at any time or for any reason. Maintain compliance with Legacy's
Compliance program and plan. Adhere to Legacy's Code of Conduct and
Dress Code. These duties are not all-inclusive but provide minimum
performance expectations which will be reviewed in the evaluation
process. Supervisor may edit, alter, add to, or exclude duties and
responsibilities based on the needs of the facility.
QUALIFICATIONS/ EXPERIENCE/ REQUIREMENTS: Minimum Qualifications
Freedom from illegal use of drugs. Freedom from use and effects of
drugs and alcohol in the workplace. Anyone found guilty by a court
of law of abusing, neglecting, or mistreating individuals in a
health care related setting are ineligible for employment in the
position. Education and/or Experience An individual with a minimum
of a bachelor's degree in social work or a bachelor's degree in a
human services field including, but not limited to, sociology,
gerontology, special education, rehabilitation counseling, and
psychology; and one year of supervised social work experience in a
health care setting working directly with individuals. Experience
in Long-Term care setting preferred but not required.
Certifications, Licenses, Registrations: For facilities with over
120 beds and/or with respiratory units, MSW may be required.
