As a Clinical Care Manager, you will oversee a team of healthcare professionals, ensuring the highest quality of care is provided. Your expertise will guide your team in facing complex patient situations, sustaining the Vital Caring Group’s commitment to enhancing patient outcomes. Assess patient needs and develop comprehensive care plans. Lead and supervise a team of healthcare professionals. Coordinate with physicians and other healthcare providers to ensure continuity of care. Monitor patient progress and adjust care plans as needed. Educate patients and their families on health maintenance and disease prevention. Ensure compliance with healthcare regulations and standards. Participate in the training and professional development of team members. Manage care team resources and schedules efficiently. Strong leadership and team management skills Excellent communication and interpersonal skills Proficiency in creating and managing comprehensive care plans Ability to make critical decisions under pressure Strong organizational and multitasking skills Knowledge of regulatory healthcare standards Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current Registered Nurse (RN) license Certified Case Manager (CCM) preferred
