Assess patient needs and develop individualized care plans Coordinate with healthcare providers to streamline care and interventions Monitor patient progress and adjust care plans as needed Ensure compliance with healthcare regulations and standards Educate patients and families on treatment options and health management Facilitate patient access to appropriate healthcare services Document all patient interactions and interventions accurately Strong interpersonal and communication skills Ability to manage multiple cases effectively In-depth knowledge of healthcare regulations and policies Proficient in medical software and patient management systems Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Certified Case Manager (CCM) preferred Licensed Registered Nurse (RN) or Licensed Social Worker (LSW) based on educational background
