Vital Caring Group, a leader in personalized home health solutions, is seeking a dedicated and compassionate Clinical Care Manager (RN) to join our team. This role involves managing patient care, developing care plans, coordinating treatment, and ensuring the highest standards of care are maintained. Our focus is on enhancing patient outcomes through personalized, comprehensive healthcare services, encouraging a holistic approach to patient health. This position requires a skilled RN who thrives in a dynamic environment and is committed to improving patient lives through effective care management and coordination. Assess patient conditions and develop individualized care plans. Collaborate with healthcare professionals to coordinate treatment plans. Implement patient care plans and monitor outcomes. Ensure compliance with healthcare laws, regulations, and standards. Provide guidance, support, and education to patients and their families. Manage a team of healthcare practitioners focusing on optimal patient outcomes. Regularly review and adjust care plans to meet patient needs and respond to changes in condition. Strong leadership and nursing skills. Excellent communication and interpersonal skills. Ability to manage multiple cases and priorities. Strong organizational and problem-solving skills. Proficiency in electronic health record (EHR) software. Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Valid RN license. Certification in Case Management (CCM) preferred.
